Central Serous Chorioretinopathy Reoccurrence in Pregnancy: Case Report and Literature Review

Central serous chorioretinopathy is a retinal disorder where areas of leakage through an altered retinal pigment epithelium cause localised serous detachment of the macula. It typically presents with blurred vision, metamorphopsia, and micropsia. While commonly affecting young men, CSCR cases have been reported in pregnancy typically within the third trimester with the incidence reported as 0.008% per year. Here we describe the case of reoccurrence of unilateral CSCR in a 32-year-old multiparous woman. She presented at 13 weeks of gestation with decreased vision in the left eye. Optical coherence tomography (OCT) revealed subfoveal neurosensory detachment with hyperreflective fibrin deposits. This was managed conservatively with lifestyle modifications and close monitoring. Vision and OCT findings gradually improved, and by 34 weeks of pregnancy, the CSCR had fully resolved. The patient successfully delivered vaginally without recurrence. Notably, she had experienced a prior episode of CSCR four years earlier, unrelated to pregnancy, which resolved spontaneously after 10 months. CSCR in pregnancy is a rare but important differential diagnosis for visual complaints in pregnancy. This case demonstrates that lifestyle modification and monitoring may lead to spontaneous pre-delivery resolution and that vaginal delivery does not necessarily worsen the condition.
